The Policy That Left the Page – A Head of AI Governance’s Story

It’s 09:06, and Nadia is standing in front of a room that contains every flavour of scepticism a governance leader can face: Legal, Security, Ops, Product, and someone from Internal Audit who has looked permanently unconvinced since 2019.
On the screen behind her is a beautifully written AI governance policy. Eighteen months of workshops, cross-functional reviews, board sign-off, and a design team that made the PDF look genuinely impressive. Nadia is proud of it. She should be. It’s comprehensive, principled, and aligned to every framework that matters.
A hand goes up. It’s always the same hand. The auditor. “This is excellent, Nadia. Quick question. Where’s the evidence that any of this is enforced?”
The room goes quiet. Not because the question is unfair. Because everyone already knows the answer. The policy exists as text, not as behaviour. It describes what should happen, not what does happen. And Nadia knows the uncomfortable truth that nobody wants to say out loud: a policy you can’t demonstrate becomes a liability the moment an incident arrives, because it creates an expectation you’ve now publicly committed to but can’t prove you met.

Nadia has been in governance long enough to recognise the pattern. Every organisation she’s worked in has had the same gap: brilliant policies, no enforcement mechanism.
The governance team writes the rules. The technology teams build the systems. And between those two activities is a void where accountability goes to die. You can win every workshop, write every framework, align to every regulation, and still get caught flat-footed when someone asks the simplest question: “Show me.”
